COYOTE VALLEY CASINOS WIN-LOSS STATEMENT

There are two easy ways to get your win-loss statement from Coyote Valley Casinos:

1. In Person for pick-up at Coyote Club Booth: To pick up your win-loss statement in person, go to the Coyote Club and show your card and valid photo I.D. A Coyote Valley Club Representative will print your win-loss statement. If you need a W2-G and/or 1099 summary, please call the casinos and ask for Debbie Logan.

2. By Email: Download, complete and email this FORM

4. By Mail: Download, print and complete this form and mail to:
